Raphael: Integrating a PalliativeCare Approach for patients with heart failure

The main ambitions of the RAPHAEL consortium are:
- To improve the quality of life, the quality of dying and the autonomy of patients with heart failure and their informal carers through an integrated palliative care approach in heart failure care, supported by home monitoring of signs, symptoms and concerns.
- To improve the cost-effectiveness of care in heart failure by reducing the number of hospital admissions and readmissions while improving quality of life.
- To provide universal and timely access to palliative care for all patients with heart failure in Europe, including those in more remote and rural areas, through a home-based, data-driven remote palliative care programme.
